more for your family? It can be exhausting—especially when you’re paying out of pocket
and still struggling to find the right support. At Heart & Home Connections, we help
families across the Triangle cut through the noise. We’ve built a trusted referral
community of vetted providers who understand that every child is different—and
that finding the right fit matters.
We support families in navigating Occupational
Therapy (OT), Physical Therapy (PT), Speech Therapy, ABA, and more. Unsure which
school setting is best? Need a knowledgeable advocate by your side? We’ll connect you
with professionals who align with your child’s unique needs and your family’s goals. You
don’t have to do this alone.
We understand the financial challenges families face when accessing services. Our
care coordination services let you build a package that works for your family,
because every family deserves care that is the right fit and financially accessible.
Families can start with a free in-home or virtual consultation and add services as
needed. We also provide a follow-up two weeks after services end to make sure your
family is successfully connected to the care you need.

My name is Christine Pontes
I am Christine Pontes, an LCSW-A based in Raleigh, NC, with over 14 years of experience in behavioral health and social services. I have supported children, adults, and families across diverse backgrounds in navigating mental health, developmental, and life challenges.

My career began working with children and adolescents as a therapeutic mentor, followed by several years supporting individuals ages 4–22 with autism and developmental disabilities in psychiatric, residential, and school-based settings. I also served as a caretaker for a child with autism and Down syndrome, which shaped my family-centered approach.
I later transitioned to working with adults as a behavioral health care manager, supporting individuals with mental health and substance use conditions, complex medical needs, and social determinants of health. In this role, I coordinated care, connected clients to community resources, and advocated to improve stability and quality of life.
I earned my Master of Social Work in 2022 and have since worked as a mental health therapist providing in-home and outpatient services. I hold provisional licensure in North Carolina, Massachusetts, and Rhode Island and take a collaborative, compassionate approach to care.
